Information, referral and outreach
about the many services and benefits available to older adults.
Information may be given by phone, in the office or at a home visit.
Assistance includes:
- Answering questions about government benefits and community
resources such as Social Security, Medicare, Medicaid, Circuit
Breaker, energy assistance and housing.
- Intervening with an agency on a client’s behalf
- Assessment of need and referral for service
- Assistance in the completion of forms and applications for
benefits.
Case Management Services
assesses the need for care, develops an overall plan and will
follow-up to see that that their client is receiving the service
needed. Case managers assist seniors with obtaining home delivered
meals, and in-home services, and assess their needs in areas such
as:
- Living arrangements
- Transportation
- Personal care Assistant
- Financial
- Nutrition
- Housekeeping and maintenance
Elder Abuse Intervention
investigates allegations of abuse, neglect and exploitation of older
persons. Appointed by the Illinois Department on Aging, caseworkers
investigate:
- Physical and emotional abuse
- Financial exploitation
- Confinement neglect
- Depravation of food, medicine
- Sexual abuse
Nursing Home Ombudsman Services
assists residents of long term care facilities and their families
by:
- Providing consultation on how to chose a nursing home
- Advocating for resident rights
- Investigating complaints
- Mediating solutions to problems
